The Case
AY is a patient in his late forties who experienced a violent assault four years ago and has lived with several challenges, for example, head injury, reduced mobility, and continence management applications. The sister wants to cut costs on the suffering patient and is confused about whether to provide home care to the brother or continue with the hospitalization.
Care Issues
The care issues for the patient in the case include emotional management of the family members who have spent a lot of cash for the last four years in the hospital. Also, the proper care away from the hospital and how it may impact the patient’s health and healing process. The patient’s mobility problem may be challenging when the patient is away from the facility. The patient will require safety measures to enhance the recovery process. The carers should ensure the safety of the individual through different methods which could improve the connection with the sister and the family. The family should devise a plan to ensure a successful transition from the hospital to a residential facility for longer care services. The patient will require mobility devices like a wheelchair to improve movement in the new environment. The patient will also require a full-time carer to monitor the situation and outline effective treatment strategies.
Follow up Plans
The patient will be suited to be in longer-term residential care equipped with different equipment and services to enable him to recover effectively and enjoy the benefits of the new approach. The facility should provide advanced care championed by qualified caregivers and nurses in the region. The caregivers should implement conducive techniques to improve the emotional stability of the individuals (Devi et al., 2022). For example, the children and the sister should have emotional support to overcome the issues in the family.
The telehealth services will enhance patient monitoring at a reduced cost compared to regular visits by physicians. The patient will be assigned a caregiver who will have close contact with the organization through the telehealth platforms to help the patient recover effectively (Dai, 2023). The approach will be cost-effective since the healthcare organization constantly connects with the residential facility. Also, using longer-term residential care will be effective for the patient since the family will have close access to the patient. At the same time, the organizations will also monitor the patient’s progress. This will reduce any errors, thus increasing the recovery rate.
Reflection
The informatics will improve the care issues since the patient’s family will closely connect with the facility. The sister and the children will provide the required details since the patient may communicate poorly. The organization will integrate the information from the patient and compare it with relevant medical knowledge to formulate the best treatment option for the individual. Informatics enables nurses to have various options in caring for the patient, thus providing the best care with reduced errors and health implications. The informatics also improves the communication stream between healthy individuals in the facility (Al Najjar et al., 2022). The nurses will provide effective and relevant information collected from the patient and related individuals to the doctors. The information helps improve patient outcomes, enhancing the facility’s quality of services. Therefore, coordinating between the sister, children, patient, and nurses will enhance information delivery to the organization, thus implementing appropriate measures to improve the outcome of the individuals.
