Introduction

This paper aims to develop a health promotion plan that will help address three wellness assessments, including lack of physical exercise and poor nutrition, lack of adequate sleep, and stress management. The purpose is to provide a comprehensive health promotion plan that will adequately assess, diagnose, plan, implement, and evaluate strategies that will help promote overall well-being. This paper aims to help contribute to personal well-being and improved health outcomes by providing a thorough health promotion plan.

I am a (age)-year-old (occupation) residing in (place). My work environment involves (description), and my home environment includes (description). This assessment plan will help provide a comprehensive health promotion plan that will help improve my well-being.

From my wellness assessment worksheet and plan assignment, I identified three key areas of concern, including lack of physical exercise and poor nutrition, lack of adequate sleep, and stress management. During the assessment, preconceived ideas about personal health were challenged, leading to the acknowledgment of neglect of personal well-being. This prompted my awareness of the importance of self-care while caring for the well-being of patients.

Three crucial nursing diagnoses were identified, each presenting strengths and weaknesses. First is lack of physical exercise and poor nutrition. Strengths included recognizing the issue and consistently exercising thrice weekly to improve overall well-being (Semeco, 2021). Challenges included having difficulties maintaining a balanced diet on weekends and the possibility of not actively adhering to the planned exercise schedule, such as missing some days of exercising due to prevalent and avoidable circumstances.

Another area of concern was the need for more adequate sleep. Strengths included establishing a consistent sleep schedule, aiming for 7-8 hours of sleep each night, and evaluating and improving the sleep environment for comfort and tranquility. While no challenges were reported, potential challenges may include sleep deficiency, poor sleep habits such as irregular sleep schedules, and increased mental health stressors such as stress and anxiety, which may result in poor sleep quality.

Moreover, another area of concern recognized in the wellness assessment worksheet was stress management. Strengths include seeking support from friends and family to help manage stress and attending RN-BSN courses to reduce stress and consistently practicing stress-reducing activities such as practicing deep breathing exercises (Robinson & Smith, 2023). Challenges include Identifying and addressing specific stressors at work and other potential difficulties in maintaining a balanced diet on weekends, contributing to increased levels of stress (Centers for Disease Control and Prevention, 2021).

The ongoing efforts on specific stressors and refining nutritional habits will contribute to sustained advancements in personal health.

In conclusion, the health promotion plan will significantly contribute to my overall well-being. While providing care for patients in healthcare institutions, it is also crucial to promote personalized self-care that will help enhance personal health and well-being. Reflecting on preconceived notions, strengths, and challenges highlights the importance of self-care. Moreover, the identified strategies, such as attending the RN-BSN course and maintaining a positive work environment free from stress, are essential for maintaining a balanced personal and occupational life.
